  2. Check payment addressed to Saint Louis University, Inc. and forwarded to Saint Louis University, School of Nursing, 5th flr, Diego Silang Building, Bonifacio St, Baguio City with the list of the names of participants, participant category (professionals or undergraduate students), and registration category (stay-out or stay-in).
