Additional Requirements for Foreign Applicants

In addition to the above, all foreign applicant must submit together with the application folder, the following documents (FOR VISA purposes). Deadline: March

1. Two (2) original copies of the transcript of records. (Duly authenticated by the Philippine Embassy in the student's country of origin).
2. Six (6) copies of the “Personal History Statement Form” duly accomplished with 2 x 2 photo on plain white background.
3. Two (2) copies of notarized Affidavit of Support with bank certificate.
4. Recommendation letters from two (2) school officials.
5. Certificate of Eligibility from CHED (c/o SLU upon submission of required documents).
6. Photocopy of passport datapage and birth certificate authenticated by the Philippine Embassy in their country.
7. Certificate of having passed the English Proficiency Test.
8. Police Clearance Certificate from student's country (Authenticated by the Philippine Embassy in the student's country of origin – with Embassy's Red Ribbon);

Note: Documents (a) (c) and (f) have to be authenticated by the Philippine Embassy in the student's country. Computer printouts are not acceptable.
