Procedure for Application of Credentials

BY E-MAIL

1. E-mail the following information to sluregis@slu.edu.ph:
   

   1. Name (for married females students please indicate maiden name)
   2. SLU ID Number
   3. Relevant information that may help in locating the applicants records such as year graduated, course, last term attended etc.
   4. How Credentials will be claimed:
      1. Personal pick-up
      2. Through a Representative
         

         Note: Representative must present Authorization Letter from the owner/applicant and a valid ID from both the representative and applicant

      3. Through Mail.

         Furnish address where the credentials will be sent to.

          

          

2. The applicant will be informed on the total fees to be paid through e-mail.
3. Payment should be made through money order payable to “ Saint Louis University ”.
4. Processing of application will start upon receipt of payment.

* Fees are based on what credentials are requested for. If credentials are to be mailed, mailing fee is based on the current postal charge which will be added to the total fee to be paid.
