Information Regarding Admission of New Pupils to
SLU-LES for School Year 2007 - 2008

FOR INCOMING KINDERGARTEN PUPILS

Application for admission is scheduled on:

Date : October 16 to December 6, 2006 EXTENDED

Time : 7:30 - 11:30 AM and 1:30 - 4:30 PM; Monday to Saturday

Venue : Registrar's Office, 2nd Flr. Fr. Ghisleen de Vos Bldg

SLU Lab. Elem. School , Gen. Luna Road

Who are qualified to apply:

1. Children who were 2001 born
2. Children who were born January to April 2002
3. Children who were born May 2002 (to be entertained with specific conditions)
4. Children who were born October to December 2000 (for those without background schooling)

Requirements to be brought:

1. Certified True Copy of Birth Certificate
2. Certified True Copy of Baptismal Certificate
3. For 2002 born: Report Card with First Grading Grades from DepEd recognized nursery or Pre-School
4. For children of SLU Employees: A Certificate of Reservation to be secured from the SLU Accounting Office

NOTES:

1. Testing Fee is Two Hundred Pesos (P 200.00)
2. Release of the List of Qualified Applicants and enrolment will start in December 2006.
3. Orientation for new parents will be given after the enrolment.

FOR TRANSFEREES (Grade 1, 2, 3, 4, 5 and 6)

1. Application for transfer are entertained only IF THERE ARE AVAILABLE SLOTS in the grade level where transfer is sought, and that the transferee PASSED THE SCHOOL'S QUALIFYING EXAMINATIONS.
2. Pupil-applicants for transfer will be entertained starting January 15, 2007 until April 13, 2007 . Late applicants may inquire about the availability of slots two working days after the enrollment.
3. Enrollment is scheduled on May 2 to 5, 2007

Requirements to be brought:

1. Form 138 or Report Card of the previous academic year, or a photo copy of the Report Card with second grading grades duly certified by the school principal.
   For Grade One Applicants: In the absence of the confirmation for promotion to Grade One, a certificate that the pupil will be promoted to Grade One after completion of his/her present grade level must also be submitted.
2. Certified True Copy of Birth Certificate.
3. Certified True Copy of Baptismal Certificate.
4. For Grades 2 to 6: A Certificate of Good Moral Character from the previous school attended and completion of SLU Laboratory Elementary School's Recommendation Form.
5. For children of SLU Employees: a Certificate of Reservation to be secured from the SLU Accounting Office.
